2. Complete the simplified enrollment form including the following information:

3. Purchase coverage with a credit card or download the mail in enrollment form
4. Once you have received an email confirmation verifying enrollment, please log onto your parent account, go to “Forms and Documents” and fill out the “Tuition Insurance” form.
